If a SNOMED clinical term has been recorded in error or is no longer clinically appropriate, you can remove it from the patient record. Removed terms are not permanently deleted, they are logically hidden but can still be viewed when needed.
Remove a SNOMED clinical term
Navigate to the relevant screen and locate the record with the SNOMED term you want to remove.
The SCT icon will be orange, indicating a term is currently recorded.
Click the orange SCT icon.
A pop up window opens, listing details of all SNOMED clinical terms currently recorded for this item.
Click the red minus (−) icon next to the term you want to remove.
The SNOMED Clinical Terms Removal screen is displayed.
Enter a removal reason in the Removal Reason field. This is mandatory. Select from the pick list provided.
Click Remove.
The term is logically removed from the patient record. The SCT icon reverts to blue, indicating no active SNOMED term is recorded for this item.
📌Note: Removed terms are not permanently deleted from the database. You can view removed terms at any time by ticking Include Removed on the SNOMED Summary screen and clicking Filter.
