The SNOMED Summary screen gives you a consolidated, chronological view of all SNOMED clinical terms recorded across all enabled areas for a patient. It is the primary tool for clinical review and handover, providing a structured, coded overview of the patient's clinical picture without needing to navigate to individual screens.
Access the SNOMED Summary screen
Navigate to the patient's record in the Clinical Portal.
Click the SNOMED Summary link, this is typically located at the bottom of the Conditions screen, or accessible directly from the Clinical Portal depending on your local configuration.
The SNOMED Summary screen is displayed, showing all SNOMED clinical terms recorded for this patient across all enabled areas.
Read the SNOMED Summary screen
Each row in the summary list shows the following information:
Column | What it shows |
Term | The SNOMED clinical term recorded. |
Source | The area of Rio where the term was recorded (e.g. Conditions, Allergies, Assessment Form). Click the Source link to navigate directly to that area. |
Date | When the term was recorded. |
User | Who recorded the term. |
Comment | Any free text comment added when the term was recorded. |
Sort the summary list
Click any column heading to sort the list by that column.
The list is sorted in ascending order by the selected column.
Click the same heading again to reverse the sort order to descending.
Filter the summary for clinical review and handover
The filter panel on the right hand side of the screen lets you focus on the most relevant information. Use the filters below depending on your clinical use case:

Filter Source | Filter by the area in Rio where the term was recorded (e.g. Conditions only, Allergies only, Assessment Forms only). | Handover: filter to Conditions and Allergies for a quick clinical summary. Ward round: filter to Assessment Forms for recent findings. |
Filter Period | Pre-set time periods (e.g. Last 7 days, Last 30 days, Last 3 months). Select Other to specify an exact date range. | Handover: filter to last 24 hours for overnight changes. Review: filter to last 3 months for CPA review preparation. |
Inpatient Episode | Filter by the date range of a selected inpatient episode. | Inpatient handover: filter to the current admission to see only terms recorded during this stay. |
Include Removed | Tick to display logically removed SNOMED terms alongside active terms. | Clinical review: include removed terms to see the full history of coded clinical information, including terms that were corrected or removed. |
Select your filter criteria from the filter options on the right-hand side of the screen.
Click Filter to apply the selected criteria.
The summary list updates to show only terms matching your filter criteria.
Click the Client Banner to return to the Clinical Portal when finished.
đ¤Tip: At the start of each shift or handover, open the SNOMED Summary for each patient and set Filter Period to Last 24 hours with Include Removed unticked. This gives a concise, structured view of all new clinical findings coded during the previous shift, without needing to read through free text progress notes.
View SNOMED details from other screens
You can also view SNOMED clinical term details from within other areas of Rio. For example, the Conditions or Diagnosis screens. Wherever you see the SCT icon, clicking on it will display the details of the recorded term without navigating to the full Summary screen.
