Rio supports dual coded diagnosis records, where an ICD-10 diagnosis can also be linked to one or more SNOMED CT clinical terms. This article explains how to add a SNOMED CT term to a diagnosis that has already been recorded, and how to remove a linked term if needed.
đNote: SNOMED CT is an optional module. If you do not see the SCT icon or SNOMED search options on the Diagnosis screen, contact your system administrator to confirm whether SNOMED CT is enabled and whether your role has been granted access.
Link a SNOMED CT term to an existing diagnosis
Use this when a diagnosis has already been recorded but has no SNOMED CT term linked to it (shown by a blue SCT icon) and you need to add a SNOMED link retrospectively.
Access the patient's Diagnosis ICD10 screen.
Click the SCT icon (blue) on the recorded diagnosis that needs to be linked to a SNOMED term.
Result: The SNOMED Clinical Terms window is displayed, showing any current SNOMED terms and options to Remove or Add Additional Terms.Click Add Additional Terms.
Result: The SNOMED Search window is displayed. The Search Result(s) column shows any concepts that map to the recorded ICD-10 diagnosis.Select an item from the Search Result(s) column.
Result: The details are displayed in the SNOMED Concept Hierarchy column. Arrow symbols indicate where the next layer can be expanded.Navigate the hierarchy using the arrow symbols to find the most specific and clinically accurate term for this patient's diagnosis.
Select the correct term from the hierarchy.
Enter any additional comments in the Comment text box if required.
Click Save.
Result: The SNOMED Search window closes and you are returned to the SNOMED Clinical Terms window.Click Close.
Result: The window closes. The SCT icon on the diagnosis changes from blue to orange, confirming that at least one SNOMED term is now linked. The SNOMED term is visible under Clinical Term within the diagnosis.
đ¤Tip: For some ICD-10 codes, your organisation may have pre-mapped a SNOMED term. In these cases, the matching SNOMED term is automatically displayed in the Search Results column when you select the ICD-10 code, you may only need to confirm the selection rather than searching manually.
Remove a linked SNOMED CT term
Access the patient's Diagnosis ICD10 screen and click the SCT icon (orange) on the relevant diagnosis.
Result: The SNOMED Clinical Terms window is displayed showing current SNOMED terms with a Remove option for each.Click Remove in the Remove column of the relevant term.
Result: The SNOMED Clinical Term Removal window opens.Select a reason for removal from the Removal Reason dropdown.
Click Save.
Result: The SNOMED term is removed. The window closes and returns to the SNOMED Clinical Terms window with confirmation of the removed term. If no SNOMED terms remain, the SCT icon reverts to blue.
