The Clinical Portal brings together key clinical history information so you can review a patient's risk assessments and recorded SNOMED terms without leaving the portal. This article explains where to find this information and how to access more detail when you need it.
View recent risk assessments
On the Safety Overview tab, the Risk & Safety Summary panel displays key risk-related information at a glance, including the patient's current risk level, last risk assessment date, legal status, and MHA status.
To see a list of the patient's most recent risk assessments in full, look for the Recent Risk Assessments panel on the Safety Overview tab. This panel shows:
The name of the risk assessment form.
The date it was completed.
The clinician who completed it.
The recorded risk level.
The most recent assessments are shown first. To view more, click the maximise icon on the panel to expand it.
⚠️Important: If the Overdue Risk Review indicator icon is displayed at the top of the Safety Overview tab, this means the patient's last risk assessment was completed more than six months ago, or no risk assessment has ever been recorded. This should be actioned promptly.
Go to the risk assessment screen
To open, complete, or review a risk assessment in full, use the Safety Navigation panel on the Safety Overview tab. Click Risk Assessments to go directly to the risk assessment screen for this patient.
View SNOMED terms
The SNOMED Summary panel on the Safety Overview tab shows SNOMED clinical terms recorded for the patient in the last 30 days. For each term, the panel shows:
The SNOMED term.
The source area where it was recorded.
The date it was recorded.
The user who recorded it.
To see a full history of SNOMED terms beyond the last 30 days, use the Safety Navigation panel and click SNOMED Summary. This takes you to the full SNOMED Summary screen where you can apply filters and view the complete term history.
📌Note: If the No Recent SNOMED Terms indicator icon is displayed at the top of the Safety Overview tab, this means no SNOMED terms have been recorded for this patient in the last 90 days.
If information is not showing
If a panel is not displaying data you expect to see, first refresh the portal by clicking the patient's name in the Client Banner. The portal only loads data when the screen is first opened, so a refresh may be needed to show the latest records.
If the issue persists after refreshing, contact your system administrator.
