The Diagnosis screen in Rio is where you record, view, and manage ICD-10 diagnoses for a patient. Understanding the key elements on this screen will help you record diagnoses accurately and interpret a patient's diagnosis history at a glance.
đNote: Your organisation's system administrator controls which event types are available for diagnosis confirmation. If you cannot see an expected option, contact your system administrator.
Key screen elements
Diagnosis list
The diagnosis list shows all current diagnoses recorded for the patient. Each row displays the ICD-10 code, description, date of diagnosis, and the number of clinical events the diagnosis has been linked to.
Linked Event column
The Linked Event column tells you the confirmation status of each diagnosis:
0: The diagnosis is unconfirmed. It has been recorded but not yet linked to a specific clinical event.
1 or more: The diagnosis is confirmed. It has been linked to one or more clinical events (such as a CPA Review, referral, or inpatient episode).
SCT icon
The SCT icon indicates whether a SNOMED CT clinical term has been linked to the diagnosis:
Blue SCT icon : No SNOMED CT term has been linked to this diagnosis.
Orange SCT icon: At least one SNOMED CT term has been linked to this diagnosis.
đNote: SNOMED CT is an optional module. If you do not see the SCT icon or SNOMED search options, contact your system administrator to confirm whether SNOMED CT is enabled for your organisation.
Primary diagnosis
The primary diagnosis is the top entry in the Selected Diagnosis column. It is highlighted in green when a clinical event is selected. You can change the primary diagnosis by dragging it to the top of the list.
Show Removed Diagnoses
Clicking Show Removed Diagnoses opens the Diagnosis History screen, which displays both current and previously removed diagnoses. Click the + icon next to a removed diagnosis to expand its removal details, including the reason, date, and the user who removed it.
Diagnosis types at a glance
There are three states a diagnosis can be in:
Unconfirmed: Recorded without being linked to a clinical event. The Linked Event column shows 0. Can be confirmed later.
Confirmed: Linked to a specific clinical event (such as a Finished Consultant Episode, CPA Review, or referral). The Linked Event column shows 1 or more. Once saved to an event, additional details cannot be changed.
SNOMED linked: An ICD-10 diagnosis that also has one or more SNOMED CT terms linked to it. Shown with an orange SCT icon.
â ď¸Important: Once a confirmed diagnosis has been saved to a clinical event, its additional details cannot be edited. Always verify all details before saving.
