The Clinical Portal is the main screen you use to view a patient's safety and clinical information in Rio. It is designed to surface the most important information, such as active alerts, risk flags, and care plan actions quickly and clearly, so you can make informed clinical decisions with confidence.
This article explains how the portal is structured and what each part of the screen does.
How the portal is structured
The Clinical Portal is built on a three level hierarchy: the portal itself, tabs called Clinical Summaries, and panels within each tab.
The Client Banner
At the very top of the screen, the Client Banner is always visible regardless of which tab you are on. It displays key identification details for the patient, including their name, date of birth, NHS number, care coordinator, and team.
Clinical Summary tabs
Below the Client Banner, you will see a row of colour coded tabs. Each tab is called a Clinical Summary and contains a different set of information. For example:
The Safety Overview tab (shown in red) contains safety critical information such as alerts, risk flags, and care plan actions. This tab is always visible and cannot be closed.
The Care & Treatment tab contains care plans, recent assessments, and progress notes.
The Clinical History tab contains diagnosis information, appointment history, and risk score trends.
⚠️Important: The Safety Overview tab is always the first tab displayed and cannot be closed. This ensures safety critical information is always visible when you open a patient's record.
Panels
Within each tab, information is displayed in sections called panels. Each panel shows a specific type of clinical data. The panels you see on the Safety Overview tab are designed to give you an immediate picture of the patient's safety status.
What you will see on the Safety Overview tab
The Safety Overview tab brings together the most important safety information in one place. Here is what each panel shows:
Safety Indicators
A row of icons at the top of the tab. Each icon represents a specific safety flag. For example, active alerts, allergies, conditions, or consent status. If a bespoke indicator is triggered such as an overdue risk review or open care plan action, an additional icon will appear. Click any icon to see more detail.
Risk & Safety Summary
A panel showing key risk related information from the patient record, such as their current risk level, legal status, MHA status, last risk assessment date, and care coordinator. Some fields include links that take you directly to the relevant screen.
Outstanding Care Plan Actions
A table showing active care plan interventions, including the responsible clinician, due date, and current status.
SNOMED Summary
A table showing SNOMED clinical terms recorded for the patient in the last 30 days, including the source area and date recorded.
Active Alerts List
A table showing all active alerts for the patient, including severity, date added, and review due date.
Safety Navigation
A panel of quick links giving you one click access to key safety screens, including Active Alerts, Risk Assessments, Care Plans, SNOMED Summary, Safeguarding, and MHA Administration.
Refreshing the portal
The information displayed in the Clinical Portal is loaded when you first open the screen. If another member of staff adds a new alert or updates a risk record while you have the portal open, those changes will not appear automatically.
⚠️Important: Always refresh the portal before making a clinical decision, after being notified of a new alert or risk update, and at the start of each clinical contact.
To refresh the patient's Clinical Portal, click the patient's name in the Client Banner. This reloads the screen and displays the latest information.
Safety pop up notices
For some patients, a safety notice may appear automatically when you open their Clinical Portal. This is a full screen pop up that you must read and dismiss before you can access any other part of the portal. It is used for high risk patients, safeguarding cases, or patients with specific safety requirements.
If no safety notice is recorded for the patient, the pop up will not appear.
