This is a known issue and fixed in RIO version 24.04.00.
If you experience this issue, type talk to a person and the Digital Assistant will put you through to a live support agent.

Deleting or updating an assessment form via Clinical Record Management only updates the original form, not the updated versions. The updates do not appear when searching via Clinical Record Management.
