Unable to access client record deleted at a national level

Guidance when trying to access a record the following error message appears.

Error message:

The demographic record for this client has been deleted at a national level. Please contact your system administrator or local back office for assistance.

The client may have a new identity. In this case, we recommend:

  1. Creating a new client record in RIO.

  2. Sync the new client to the new NHS number.

  3. Manually move any required date from the deleted client to the new client.

⚠️Important: Some organisations choose to merge the old client into the new client. We do not advise this approach, as details of the previous identity such as name, gender or parent information may still appear in historical notes within RIO.


If the NHS number has not changed

If the National Back Office confirms that the client's NHS number has not been deleted:

  • Please type talk to a person and the Digital Assistant will put you through to a live support agent.

  • Provide the ClientID in question.
