The Assessments tab on the Consultation Screen gives you access to assessment forms without leaving the outcome workflow. Forms are drawn from your Rio menu and open in a pop up window, each one is saved independently as you go.
đNote: An assessment is not mandatory. If you do not need to record one, you can save the Consultation Screen without visiting the Assessments tab.
Record an assessment form
Click the Assessments tab.
Result: The Assessments tab is displayed, showing all assessment forms available to you via your Rio menu, grouped by menu name. If no forms are displayed, contact your System Administrator, the required forms may not be on your Rio menu.Locate the required assessment form in the list. Forms are grouped by their menu name, expand the relevant group if needed.
Click the assessment form name.
Result: The assessment form opens in a pop up window.Complete the assessment form as required.
Click Save within the pop up window.
Result: The assessment form is saved. The pop up window closes and you are returned to the Consultation Screen.Repeat for any additional assessment forms required.
How assessments differ from progress notes
It is important to understand how assessment forms behave differently from progress notes on the Consultation Screen:
Assessments are saved independently, each form is saved when you click Save within its pop up window, not when you click Save on the Consultation Screen.
Assessments are not formally linked to the appointment, there is no data model link between an assessment form and the appointment outcome it was recorded from. The assessment is saved as a standalone record against the client.
Assessments are not affected by outcome reversals, if you later reverse the appointment outcome, assessment forms recorded on the Assessments tab are not affected.
đNote: If you need to reference an assessment in the context of the appointment, include a note in the Outcome Comment or Progress Note fields.
