Troubleshoot SNOMED CT issues

Solutions to common SNOMED CT problems you may encounter as a clinical user.

If you experience an issue with SNOMED CT in Rio EPR, the guidance below covers the most common problems and their resolutions. Some issues can be resolved by you directly; others will require your System Administrator to investigate.


The SNOMED icon is not visible on a screen

Likely cause: SNOMED CT has not been enabled for that area, or your role has not been granted access to it.

What to do: Contact your System Administrator and ask them to check that:

  • The relevant area is enabled on the SNOMED Access page.

  • Your role is included in the access list for that area.


The SNOMED browser returns no results

Likely cause: The search term does not match any terms within the concept types or subsets configured for your area, or the configuration is too restrictive.

What to do:

  • Try a broader or alternative search term.

  • If the problem persists, contact your System Administrator, they may need to review the concept type mapping for the area.


Too many results are returned and the search is slow

Likely cause: No concept type mapping has been configured for the area, so the browser is searching the entire SNOMED database.

What to do: Contact your System Administrator, they will need to configure concept type mapping for the area to restrict results to clinically relevant terms.


You cannot change the filter in the SNOMED browser

Likely cause: The Read-Only Filter option is enabled for that area. This is intentional and prevents users from changing to non-clinical concept types.

What to do: If you believe the filter needs to be changed for clinical reasons, contact your System Administrator.


A SNOMED term is not appearing in the SNOMED Summary after saving

Likely cause: The form was closed without saving after the term was added. SNOMED terms are only saved when the main form is saved.

What to do: Re-open the form, add the term again using the SNOMED browser, and click Save on the main form before closing.


Removed terms are not visible in the SNOMED Summary

Likely cause: The Include Removed filter is not ticked.

What to do:

  1. Open the SNOMED Summary screen for the patient.

  2. Tick the Include Removed checkbox in the filter panel.

  3. Click Filter.

📌Note: Removed terms are never permanently deleted. They are always retrievable using the Include Removed filter.


Coding Assist suggestions are not appearing on Progress Notes

Likely cause: The SNOMED Coding Assist module (Patient Insight) is either not activated for your organisation, or the coding service has stopped running.

What to do: Contact your System Administrator. This issue cannot be resolved by clinical users directly.

⚠️Important: SNOMED Coding Assist is a separate optional module that requires its own activation. If your organisation has not activated it, suggestions will not appear regardless of other SNOMED settings.
