Problems, interventions, and care plans should be formally closed when they are resolved, completed, or no longer relevant. Closing them with an appropriate outcome ensures a complete audit trail and, where applicable, triggers automatic pathway action completion.
đNote: You must close all interventions linked to a problem before you can close the problem itself. Similarly, ensure all problems and interventions are closed before closing the care plan.
Close an intervention
Find the intervention you want to close, then click Close.
The Close Intervention screen is displayed.
Select an outcome from the Intervention Outcome pick list.
Enter a comment in the free text field explaining the outcome or reason for closure (up to 500 characters).
Enter the End Date. The date the intervention was completed or closed.
Click Save.
The intervention is closed. If your organisation uses ePathways and the linked pathway action is configured to complete on intervention closure, it is automatically signed off at this point.
Close a problem or need
Ensure all interventions linked to the problem are closed first. A problem cannot be closed while it has open interventions.
Find the problem or need, then click Close.
The Close Problem/Need screen is displayed.
Select an outcome from the Problem Outcome pick list.
Enter the End Date and any relevant comments.
Click Save.
The problem or need is closed.
đ¤Tip: If a referral associated with an intervention is closed, a warning message will appear the next time you view the care plan, prompting you to close the associated intervention. Always close interventions before closing a referral to avoid this warning.
Close a care plan
â ď¸Warning: Closing a care plan is permanent and cannot be undone. Only close a care plan when it is no longer relevant.
Navigate to the Care Plan Close menu option.
The exact location depends on your local configuration.
Locate the appropriate patient record using the standard search.
Select the care plan you wish to close from the list of current and past care plans.
Click Signoff & close.
The care plan is permanently closed. This action cannot be reversed.
Print a care plan
If your System Administrator has linked an Editable Letter to the care plan type, a Print or Printable Care Plan option is available. This generates a formatted version of the care plan using the letter template, which can be printed or saved as a PDF for sharing with the patient or other professionals.
