Care plan troubleshooting

Solutions to common issues you may encounter when using care plans.

If you run into a problem when creating, updating, or closing a care plan, the information below covers the most common issues and how to resolve them.


Care plan menu option is not visible

You may not be able to see the care plan menu option if:

  • The patient does not have an open referral in the correct specialty.

  • Your user account does not have menu access to care plans.

Ensure the patient has an open referral in the correct specialty. If the menu option is still not visible, contact your System Administrator to check your menu access configuration.


A pick list is empty

If a pick list such as Problem Category, Problem Type, Intervention Category, or Intervention Type shows no options, this is usually because:

  • The relevant master table entries have not been created.

  • The entries have not been mapped to the correct care plan type.

Contact your System Administrator to populate the relevant master table and ensure the mappings are configured correctly.


The Care Pathway pick list does not show an RTT pathway

The Referral to Treatment (RTT) pathway will only appear in the Care Pathway pick list if:

  • The intervention has been linked to a referral.

  • That referral has an RTT pathway associated with it.

Ensure the intervention is linked to the correct referral, then check that the referral has an RTT pathway associated with it via the referral details screen.


A pathway action is not being automatically completed

If a pathway action is not being signed off automatically when you save or close an intervention, check the following:

  • The Care Pathway field on the intervention matches the pathway action's configuration.

  • The intervention has been linked to the correct referral.

Contact your System Administrator to verify the pathway action configuration flags in particular, whether the action requires the intervention to be explicitly linked, closed, or validated before it can be signed off.


A pick list reverts to "Please Select" when editing

If a Problem Type or Intervention Type pick list reverts to "Please Select" when you edit a record, the value that was previously selected may have been removed or remapped by your System Administrator since the record was created. You will not be able to retain the previous value, you must select a new value from the list.

Contact your System Administrator if this occurs. They can review the mapping configuration to help prevent this from happening to future records.


Cannot close a problem

A problem cannot be closed while it has open interventions linked to it. Close all interventions linked to the problem first, then attempt to close the problem again.


A referral closure warning is appearing

This warning appears when a referral associated with an intervention has been closed without the intervention being closed first. To resolve this, close the intervention with an appropriate outcome. To avoid this warning in future, always close interventions before closing a referral.
