Add a problem or need to a care plan

Add a clinical problem or need to a patient's care plan, including how to use library items.

A problem or need represents the clinical issue or goal you are addressing for the patient. Each problem can have one or more interventions such as actions, treatments, or goals linked to it. You can add problems manually or select a pre-defined item from the care plan library.


Add a problem or need

  1. On the care plan screen, click Add Problem.

    • The exact label depends on your local configuration, it may appear as Add Need, Add Goal, or similar.

    • The Add Problem/Need screen is displayed.

  2. To use a pre-defined library item, click Select from Library, browse or search the library, select the appropriate item, then click Load.

    • The problem or need fields are pre-populated from the library item. You can edit any field before saving.

  3. If you are not using a library item, complete the fields manually:

    • Problem/Need Category: Select from the pick list (if configured).

    • Problem/Need Type: Select from the pick list (if configured).

    • Problem Description: Enter a free text description of the problem or need.

    • Problem Date: Enter the date the problem was identified (if configured).

    • Care Pathway: Optionally associate the problem with a care pathway.

  4. Click Save.

    • The problem or need is saved and displayed on the care plan screen.

🤓Tip: If your System Administrator has enabled problem ordering, you can drag and drop problems to change the order in which they appear on the care plan screen.
