You can create a new care plan for a patient directly from their referral record in the Clinical Portal. You might do this at the start of a new episode of care, following a Care Programme Approach (CPA) review, or whenever a new care plan is clinically indicated.
đNote: You must have an open referral for the patient in the correct specialty before you can access care plans. Contact your System Administrator if the care plan menu option is not visible.
Create a care plan
Go to the patient's record in the Clinical Portal using the standard patient search.
Select the correct referral from the patient's referral list.
The referral context is now set, and care plan options are available from the menu.
Navigate to the care plan menu option.
The exact label depends on your local configuration. It may appear as Care Plan, CPA, Goals, or similar.
The care plan screen is displayed. If the patient already has a care plan, it is shown here. If not, the screen is empty.
If more than one care plan type is configured, select the appropriate type from the pick list, then click GO.
The care plan screen is now ready. You can begin adding problems or needs, see Add a problem or need to a care plan.
