View outstanding care plan actions in the Clinical Portal

How to find and review a patient's outstanding care plan actions directly from the Clinical Portal.

The Clinical Portal gives you quick access to a patient's outstanding care plan actions without needing to navigate away from the portal. The Outstanding Care Plan Actions panel on the Safety Overview tab shows you active interventions, who is responsible, and when they are due all in one place.


Find outstanding care plan actions

To view a patient's outstanding care plan actions, open the patient's Clinical Portal. On the Safety Overview tab, locate the Outstanding Care Plan Actions panel.

The panel displays a table showing:

  • The intervention or action.

  • The responsible clinician.

  • The due date.

  • The current status.

The actions are sorted by due date, with the most overdue items appearing first.

📌Note: Only active care plan actions with no completion date are shown in this panel. Completed actions will not appear here.


View more actions

By default, the panel shows the five most recent or critical items. To see a full list of outstanding actions, click the maximise icon on the panel. This expands the panel to show up to 20 items.


Go to the care plan screen

To view, update, or complete a care plan action, use the Safety Navigation panel on the same tab. Click Care Plans to go directly to the care plan screen for this patient.

🤓Tip: If the Open Care Plan Actions safety indicator icon is displayed at the top of the Safety Overview tab, this means there are active interventions with no completion date recorded. Click the icon for a quick summary.


If the panel shows no data

If the Outstanding Care Plan Actions panel displays a message such as "No active care plan actions recorded for this patient", this means there are currently no active interventions without a completion date on this patient's record.

If you believe actions should be showing but are not, try refreshing the portal by clicking the patient's name in the Client Banner. If the issue persists, contact your system administrator.
