Understand safety indicators in the Clinical Portal

A guide to the safety indicator icons displayed at the top of the Safety Overview tab and what each one means.

When you open a patient's Clinical Portal, the first thing you will see at the top of the Safety Overview tab is a row of icons. These are called safety indicators, and they give you an immediate at a glance view of the patient's safety status without needing to open any other screen.

This article explains what each indicator means and how to use them.


Standard safety indicators

The following indicators are displayed for every patient. An icon will appear highlighted when the relevant information is present on the patient's record.

Active Alerts

Indicates that one or more active alerts are recorded for this patient. Alerts may include clinical warnings, safeguarding flags, or other important notices. Click the icon to see a list of all active alerts, including their severity and review due date.

Allergies

Indicates that one or more allergies are recorded for this patient. Click the icon to view the full list of recorded allergies.

Conditions

Indicates that one or more clinical conditions are recorded for this patient. Click the icon to view the conditions list.

Client Death

Indicates that a date of death has been recorded on this patient's record. This flag is displayed to prevent clinical contacts being recorded after a patient has died.

Consent (DCR)

Indicates the patient's consent status for the Detailed Care Record (DCR). Click the icon to view the current consent status.

Conceal from Client

Indicates that a conceal from client flag is active on this patient's record. This means certain information on the record should not be shared directly with the patient.

Local Consent

Indicates that a local consent record is present for this patient. Click the icon to view the local consent details.


Bespoke safety indicators

In addition to the standard indicators above, your organisation may have configured additional bespoke indicators. These appear alongside the standard icons and are triggered automatically based on the patient's clinical data. Common examples include:

  • Overdue Risk Review: Displayed when the patient's last risk assessment was completed more than six months ago, or when no risk assessment has ever been recorded.

  • Open Care Plan Actions: Displayed when there are active care plan interventions with no completion date recorded.

  • Active Safeguarding Concern: Displayed when an open safeguarding record exists for this patient.

  • No Recent SNOMED Terms: Displayed when no SNOMED clinical terms have been recorded in the last 90 days.

📌Note: The bespoke indicators shown on your portal may differ depending on your organisation's configuration. Contact your system administrator if you are unsure what a particular icon represents.


Clicking an indicator

You can click any safety indicator icon to open a pop up with further detail about that flag. From the pop up, you may also be able to navigate directly to the relevant clinical screen. For example, clicking the Active Alerts indicator will show you the full list of active alerts for the patient.

⚠️Important: Safety indicators are only updated when the Clinical Portal screen is first opened. If a new alert or flag has been added by a colleague while you have the portal open, it will not appear until you refresh the screen. To refresh, click the patient's name in the Client Banner.
