You can link uploaded documents to key clinical events in Rio, including referrals, admissions, discharges, appointment outcomes, and progress notes. This keeps all relevant documentation together and makes it easier to find documents in context.
Before you start
The document must be marked as Final before it can be linked to a clinical event. Draft documents do not appear in the pick list.
You need the correct permissions to edit the clinical event you want to link to.
How to link a document
Link a document to a referral
Open the patient record and go to the Referrals screen.
Open the referral you want to link the document to.
Scroll to the Associated Documents section.
Click Add.
Select the document from the pick list.
Click Save.
Link a document to an admission
Open the patient record and go to the Admissions screen.
Open the admission record.
Scroll to the Associated Documents section.
Click Add.
Select the document from the pick list.
Click Save.
Link a document to a discharge
Open the patient record and go to the Discharges screen.
Open the discharge record.
Scroll to the Associated Documents section.
Click Add.
Select the document from the pick list.
Click Save.
Link a document to an appointment outcome
Open the patient record and go to the Appointments screen.
Open the appointment and select Outcome.
Scroll to the Associated Documents section.
Click Add.
Select the document from the pick list.
Click Save.
Linking a document to a progress note
Open the patient record and go to the Progress Notes screen.
Open the progress note you want to link the document to.
Scroll to the Associated Documents section.
Click Add.
Select the document from the pick list.
Click Save.
Removing a document link
Removing a link does not delete the document — it only removes the association between the document and the clinical event. The document remains in the patient record.
Open the clinical event the document is linked to.
Scroll to the Associated Documents section.
Click Remove next to the document you want to unlink.
Confirm the action when prompted.
⚠️ Important: If a document has been locked, you cannot remove the link. Contact your system administrator if you need to make changes to a locked document.
Document not appearing in the pick list?
If a document does not appear when you click Add, check the following:
The document is marked as Final (not Draft).
The document has been uploaded to the correct patient record.
You have the correct permissions to view that document type.
