When accessing the form, you may encounter the following error:

This error can occur due to one of the following reasons:
Incomplete fields in Assessment Maintenance.
One or more configurable fields have not been completed.
Incorrect SQL queries or read-only fields.
Fields using a Generic Select List or Assessment Read only fields may be incorrectly configured.
Resolution
Check Assessment Maintenance:
Click on Assessment Maintenance from the RIO menu.
Select the Form radio button.
Scroll to the form and single-click the row.
Click Modify.
Scroll through the list of fields:
Ensure all configurable fields have a green tick.
If not:
Open the field.
Configure and select Use Format.
Save the form.
Validate SQL queries and read only fields:
Review any fields using:
Generic Select Lists
Assessment Read-Only fields
Correct any incorrect configurations.
