The Genders are hard coded into RIO, this would be required to be raised on Product Enhancement if you are requiring more Genders to be aligned with NHS Gender identity.
This has already been raised on the Product Enhancement Portal, with updates that can be provided: https://theaccessgroup-accessrioepr.ideas.aha.io/ideas/HC-I-110.
