When patient records are merged, all medications on the merged record are discontinued. This is working as designed.
Why does this happen?
Discontinuing all medications is the safest approach during a patient record merge. This prevents incorrect or duplicate medications from remaining active, which could pose a patient safety risk.
What you may see
As a result, some discontinued medication entries on the merged record may appear incorrect. This is expected, having incorrect discontinued medications visible is far safer than risking incorrect active medications.
Next steps
Re-prescribe all medications correctly on the merged patient record after completing the merge.
